Transition & Aftercare Program
Purpose
Children who come to stay at the “Home” arrive from a variety of backgrounds and with a varied and unique set of changes that need to take place before they are able to become a successful member of our society. This road to success is loaded with both opportunities and obstacles. It is a difficult path to follow and after changes are made residents will need to continue to receive support in both short and long term goals.
The Transition & Aftercare Program continues Cathedral Home’s mission by further facilitating a sense of belonging, autonomy, mastery, and hope. The Transition & Aftercare Program is designed to help ensure that the youth have all of the necessary skills to be successful once completing their treatment and that there is still support for these youth once they leave the Cathedral Home for Children campus.
This program is designed to successfully transition youth from one life stage to another, specifically from treatment to home or independent living. Transitioning a youth home can be to their nuclear family, extended family, or foster family. Transitioning a youth into independent living may also include transitioning to either the Harvey or Biegert House. Transition program will also include the development of individual plans with a focus on career education, supervised experiences and assistance in the development of career goals.
