Program Goals
Aftercare and transition implementation of program goals
- Within the first 30 days of placement residents will complete individualized assessments which will assist the team in the development of transition goals for the areas of: education, career management, projected placements, and other resources based on individual needs.
- Team participation in the formulation of specific goals to assist residents in the transfer between life stages while under our care. The transition plans will be reviewed every three months and will be adjusted to fit the current and projected needs of the resident.
- Transition to Aftercare services will begin 6 months before projected discharge from the treatment side of the program. The 6 month plan will consist of current level of performance (i.e., treatment, educational, career management, etc.). Formulation of projected Aftercare needs including educational, family, independent living, career management, and continued counseling or group needs. Specific goals to address Aftercare needs including personnel or agencies responsible for completion of goals.
- Transition to Aftercare services will occur 90 days prior to discharge from residential treatment to Aftercare services. Team will discuss current level of performance (i.e., treatment, educational, career management, etc.). Formulation of projected Aftercare needs including educational, family, independent living, career management, and continued counseling or group needs. Specific goals to address Aftercare needs including personnel or agencies.
Aftercare services will provide services for individuals graduating from Cathedral Home for Children for one year. Residents will assist in the development of Aftercare goals, including support for the areas of education, family, independent living, career management, and other resources based on the individual’s needs.Â
