Outreach Program

Maintaining a Life Time Commitment to Former Residents

Once residents graduate from Cathedral Home, they become part of the Outreach Program. All Cathedral Home staff are dedicated to students of the Home, whether enrolled or graduated. The Outreach Coordinator maintains contact, usually in person, with over 350 former residents; over 100 on a regular basis and 50 to 200 periodically.

After youth leave Cathedral Home, they may be returned to their homes, placed with foster families, in other treatment facilities, or may live independently. The coordinator provides services tailored to each person. The services, however, all follow the same model:

T - Transition from residential treatment to placement
L - Liaison with families, schools, state and agencies.
C - Contact between former residents and CHC

The following services may be provided:

  • periodic visits in the home to verify stability and success
  • family counseling on-site and in the home
  • assistance with job placement
  • coordination with local schools to address special needs
  • follow-up coordination with local medical services
  • monitoring of nutrition
  • assistance to new mothers
  • counseling for foster placement
  • assistance with housing
  • assistance with independent living (helping with furniture, etc.)
  • assistance with further education and training, scholarships
  • contact with service/assistance providers statewide

This service is provided free of charge for Wyoming residents and a fee is negotiable for out-of-state placement.
